It would be good to allow blank TOC entries to help structure the list. Alternatively, could we add a tickbox 'end of group' (or something) to the entry properties which would create a gap after the entry?

I think allowing proper groupings may be the best solution.

that's fine - groups will have some bottom margin, or how will they become visible?
It would also be helpful to be able to scroll in the contents menu. The last entries does now dip under the lower end of my screen, and are only accessible by zooming out the browser window.
Or even allowing to open or close separate groups of entries in the menu? One group for the English version, one group for the Norwegian?
very good point about the different language versions - given that I always aimed at multi-lingual expositions, it should be possible to implement it on this level too in particular if an exposition has many page and hence all pages twice or even more often...
